Maybe a spoil for those who haven't finished Portal yet:

GLaDOS's shriek at the end of the game always gives me chills. But then I remember the credits song and everything is okay :D

Yeah, definitely have to go with Amnesia. Specifically, when you find out why you wiped your own memory. And that "Paint the body, cut the lines" Flashback.

And the big sister screeching at you in Bioshock 2.

Agreed. *shivers* I still remember playing that when I was supposed to be in bed, I had a flashlight and my old gameboy under the covers at night. Scary stuff.

The flood was pretty gnarly too the first time in Halo 1.

By far the scariest thing was quite a number of moments from the first Dead Space, and I have yet to play the second, L4D has had some creepy ones too. At least to me that game gets scary sometimes.

In Sonic Adventure Battle 2 or WHATEVER it's called, when I was little there was this Ghost boss that used to scare the shit out of me, because I could never beat it and it always bursted around corners.

Pretty much any game where I look away for a second and continue walking forward, then return to realize I walked directly into a cave/hallway/room filled with enemies...

whenever you escape from the nemesis in resident evil 3, you get this eerie music that tells you he is still nearby. then when the door is out of sight he comes for you. its just that moment of peace with eerie music, you know you have to face it but you have to take a deep breath before doing so.

Doesn't still haunt me, but it did for awhile: The two nightmare sequences in the original Max Payne. The shriek of your wife, the cry of your baby all around you. Even more unsettling was the second nightmare when Max gives this horrific scream if you fall off the lines of blood.

Granted, that was one of the most annoying fucking parts of the game, but the first time I fell off and he screamed, I nearly crapped myself. Keep in mind I had the volume up pretty high and was playing late at night.
