I'm surprised no one said Tarkovsky's STALKER(doesn't resembles the games at all). Another couple of mind fucking movies are Gummo, Stay (from what I've seen), Trois Couleur (trilogy. It seems fucked up though I have seen only the first part, Blue). These are without repeating what other people said. Also you've increased my to-watch list by a pretty big ammount.

Inland Empire. I don't know what the hell was going on in that film, but whatever it was, it definitely wasn't good. I agree with all those who said Requiem for a Dream as well, I don't know I'll ever bring myself to watch it again, though. I've got Audition on DVD on my shelf, but I haven't seen it yet. Maybe I'll give it a go this evening.

Teeth

Labyrinth (dance, magic, dance!)

The Exorcist (the stair-walking scene was AWFUL)

Mother of Tears (the spear-raping scene, the guy screaming over the phone, the women getting her eyes gouged out ... woo ...)

Pretty much every Dario Argento movie. http://www.youtube.com/watch?v=Eq-IXcwZYBo

GoodBye, Lenin!

It's a comedy film, set in East Germany. Yes, I know comedies are not supposed to disturb, but some of this made me feel quite ...weird. I suppose you can classify that as "A mild feeling of disturbance".

'TideLand'.

It seemed to be disturbing just for the sake of it. Terrible storyline, annoying characters and just.. Stupid, really. But still disturbing.
Oh my god yes....this movie creeped me out, and it was made around my hometown.

I have never been truly disturbed by a film but the only thing that has come close is Begotten.
I dare you to watch this all the way through.

It pleases me others have seen this treasure.

So yeah, most everyone has beat me to the obvious choice favorites, but here we go:

- Irreversible
- Tetsuo: Iron Man
- Ichi The Killer
- Audition
- Vistor Q
- Old Boy
- Uzumaki
- Eraserhead
- Lost Highway
- Blue Velvet
- Videodrome
- Twelve Monkeys
- Cube
- eXistenZ
- Begotten
- DrumStruck
- Hour of the Wolf
- Faces of Death / Traces of Death / Guniea Pig series / Psuedosnuff
- Skinny Puppy's Too Dark Park Tour Backing Film
- SPK's Despair
- Psychic TV's First Transmission
- I Stand Alone
- No Country For Old Men
- Children of Men
- There Will Be Blood
- Jacob's Ladder
- Altered States
- Straw Dogs
- Tideland
- Caligula
- Brazil

I'm proud to say that I own all the above entries on a mix of VHS, DVD and video files where legitimate purchase was unavailable (mostly the music video / concert bootleg stuff). I'd recommend them to anyone as 'disturbing' movies, for varying reasons.
Impressive collection there, Uzumaki! Is that the spiral one? If so it is pretty messed up!

How did you get hold of the Guinea Pig series? I've been looking for that for a while!

I've seen the first Faces Of Death and then odd bits of the others, if you are a fan you should watched "Banned from TV" it is rather horrific and all real compared to some of the face stuff I've seen on Faces Of Death. Plus the music and narration just add to the experience.

Been said a lot, but Requiem for a Dream is a complete mindfuck. One of my favourite films too.

Audition is pretty bizarre (but incredible), Perfect Blue is a nasty beauty of an anime and Tetsuo : The Iron Man....well that's just pretty unique.

I'm glad someone mentioned Dario Argento movies, incredible, Suspiria is my favourite.

EDIT: I didn't want to admit it earlier, but I've seen several of the Guinea Pig series of films from Japan. Google them if you're not sure. Disgusting, pointless and extremely disturbing.

I remember the movies with the puppets one uses for comedy shows (you can put your hands in them, control them, and make them 'talk'), in which they turned evil and started to kill people. I was about 10 or 11 then, and I totally freaked out at them.
