ARMA 2.

THE most realistic shooter I've ever played. Also it's boring as hell. Running 10 minutes through a forest, getting to a small village, recieving a bullet from an enemy who's a mile away, then waiting 5 minutes until medic arrives and listening to those HORRIBLE VOICES is not for me.

Far Cry 2 deserves a mention too, but the realism was ruined by guards, who respawn as soon as the checkpoint is out of your view, brand new guns, which jam after a couple of gunfights, buddies, who appear out of nowhere just in time to save you, weapon convoys, which drive in circles and continue driving the same route after being attacked, gun storages, which work like vending machines, the fact that you recieve a phone call right after you've done a mission (how the hell can the caller know what I just did?) and the fact that your character doesn't speak a single word during the game and still manages to make friends.

Rainbow Six Vegas was also pretty realistic and actually fun. Take note, ARMA.
Honestly, Far Cry 2 would have been very good with a few tweaks. First, if:

* Weapon convoys moved from point to point resupplying along the way.

* Guards at cleared Checkpoints don't respawn, more soldiers are sent from nearby bases.

* If faction progression actually changed the disposition with that faction. (IE as you progress along one faction's story missions, they become friendly, you gain the ability to call in their forces to secure checkpoints, ect.

* Location based damage. No mobile styrets.

* Some kind of STALKER Clear Sky style faction warfare would have gone a long way towards making the setting feel like a civil war, rather than just a bunch of guys waiting at checkpoints for YOU to come along.

* The weapon system in FC2 was f---ed across the board. I like how the weapons deteriorate visually, and wouldn't even care if that happened as fast as it does if it was exclusively aesthetic. But, that a brand new H&K G3 goes from mint condition to exploding in your hand in less than 24 hours is just plain rediculous. There's no weapon mod system, so, no scoping your G3 or slapping a silencer on the AK47. No high capacity handguns (SIG 226s, H&K USPs, Glocks, ect). Generally I wouldn't harp on this, (weapon upgrade schemes have been over used in games lately), except the additional ammo and durability upgrades seem kind of odd when there's nothing else.

Sorry, I'm ranting, I'll stop now.

most realistic game ever? AIRSOFT
Hell no. Mainly because there's not enough incentive not to get hit. A game of incredibly unsafe extreme woodsball (velocity cranked up all the way on all the guns, ballistic eyepro mandatory, gas masks with no canister preferable, no minimum range, and limited hand-to-hand allowed) with an OUTSTANDING group of referees (to call vital hits and armor shots) and a nice group of extremely well-geared players beats the hell out of airsoft.
so what your saying is that you have no idea how real airsoft is played (IE, with humvees, blackhawks (REAL ONES) duce and a halfs ect) and you just like to make a mess with unrealistic looking guns?

Operation: Flashpoint and the ARMA series as far as FPSs go.
Second. And just wait for Operation Flashpoint: Dragon Rising. If what they say is true, this game will surpass the rest.

Steel Battalion is supposed to be realistic to the point of ridiculousness. It's a game where you pilot a giant mecha suit. Sounds like something that doesn't exactly lend itself to realism, right? Wrong! Sit down!

First and foremost, there's the controller.

Yeah. Feeling immersed yet? Good!

All those fancy knobs and levers? They've all got a purpose. Playing Steel Battalion isn't as simple as turning the game on and going; you've got to memorize and execute a complicated boot sequence, the kind you'd probably have to memorize when starting a plane. Then, only after you've powered up your mech, do you get to move around and 'splode stuff.

But the realism doesn't stop there. If you find yourself in a tight spot, you'll be given multiple prompts to eject. After all, if your mecha's about to explode, it only makes sense that you'd want to get out in time, right? Well, here's the kicker. If you try to pull a George Kirk (Captain Kirk's father. Sacrificed himself to... Ah, forget it) and pilot your giant mecha through to the explosive end, your character is killed... And your save file is erased, effectively mimicking what real-life death would mean for you.

How's that for realism?
Thats gotta be the sexiest post i've seen on the internet.

Probably Forza 2. At least until Forza 3 and Gran Turismo 5 come out.
It's obviously not going to be a shooter, because they all either let you instantly heal via health packs or give you regenerative powers.
Sports games are where it's at, though most aren't quite there yet, FIFA 10 is close but still not quite. So it must be in motorsport and the best example I can think of at the moment is Forza 2.

Robinson's Requiem - a DOS game, where you basically roleplayed Robinson Crusoe.
Roleplaying includes dying from various diseases, starving to death, getting eaten by a grue, making your own food, medicine and stuff. Basically game about survival.
Its realism was its main flaw, as the game was incredibly hard and buggy.

Other realistic games:
Any good flight simulator(like Falcon, Il-2, MS Flight Simulator)
Operation Flashpoint and ArmA series
Any sci-fi simulator that includes Newtonian Physics(like Frontier)

EDIT: Steel Panthers, Close Combat 1-5(not that FPS bullshit), Red Orchestra

The Total War games. Not the campaigns so much, but the battles are ridiculously realistic (but still very fun). Soldiers get tired and slow down over time! I mean, come on!

Far Cry 2 was pretty realistic, though it was therefore rendered incredibly boring.
If you think guns are that inaccurate, I have some bad news for you.
I'm not sure how you deduced from my comment that I was saying the guns were innacurate.

I was referring to the general feel of the game, how most actions stayed in a first person view, such as healing, entering buildings etc...

Plus it was realistic in the way that real life can be boring as hell...

...like Far Cry 2.
