The way the question is posed is ridiculous. Most realistic game but not a simulator? All games are simulators, some more realistic than others. The answer depends on exactly one thing: where you draw the line of realism where the game becomes a "simulator for realz". Which is to say, the answer is totally arbitrary.

I agree. Rainbow Six was probably the most realistic game i've played. And I liked the healing thing in Farcry. Ripping wire out of your arms and snapping your limbs back in place. Awesome.

dammit. I know i'm behind the times (going back playing games i missed), but i seriously JUST started Indigo Prophecy. I know i'm just bitchin, but isn't that why the spoilers are even an option?

A HL2 mod recently came out, called Neotokyo. Despite its Ghost in the Shell/Akira-esque sci-fi setting, it manages to be very realistic, for several reasons:
-No crosshair unless you aim with the iron sights.
-No class can withstand more than ten bullets with any weapon, and the lightly armored Recons can't take more than three or four.
-Bullets travel through thin surfaces such as sheet metal, wood paneling, and glass.
-Guns don't sound like thunderstorms.
-Reloading your weapon also discards any ammo remaining in the magazine.

There are probably a few other things that I forgot, but overall these points are what make the game so realistic. It's rather nice, really, because it encourages teamwork, strategy, and good communication. You can't just run in, guns blazing, if you don't want someone to blow your head off with an AA13 shell through the wall.

If you weed out the obvious armor and camouflage system, while close to technology that currently exists CRYSIS is pretty realistic as well. It holds to its realism up until the plot twist 3/4 of the way through the game,

My vote is probably for Call of Duty Hardcore also. The only way they really could have made that more realistic is if instead of getting radar someone TOLD you where the enemies were at. Then again, it'd probably get really annoying hearing "He's in the market place! or He's hiding under the car to the right!" everytime you got a UAV.
Dead on, both of them, maybe make your avatar a little more sluggish and weighty after they sprint 200 yards, increase the amount of ammuntion and grenades carried, and replace the sound designers with the guys from Battlefield: Bad Company (most realistic sounding battlefield environment EVER), reduce the resilience of the helos (seriously, you can bug a chopper out by nailing his engines with sustained gunsfire), and make the body armor the avatars are wearing actually do something. I don't believe I've ever seen a game realistically implement body armor. Oh well, there's my $.02.
