There were several things.
The scripting of companions was clunky at best, broken at worst.
I ran into more glitchy terrain than I could keep track of.
Dialogues needed to be done in proper order, and it wasn't clear which order that was.
The inventory system...every 3 minutes I had to stop to sort my bags, equip upgrades, spread my old gear among companions, keep track of what to keep for those companions at the base, junk stuff I didn't need...just too much micro-management.
Plot items never disappeared. Even after their usefulness had passed.
Combat had 6 attack buttons, most characters had more than 6 attacks I wanted bound there.
In spite of this I really liked the game. I am hopeful that Bioware developers will do to this game what they did to Mass Effect. Narrow it down, maybe get rid of the inventory all together, narrow down the talent trees, tighten up the combat system and release a new and improved game. It is Bioware, and that brings a lot of hope.
