Ninja gaiden or DMC4:pC legendary dark knight or hell and hell mode are the hardest things i can think of. (no jokes on that please)

I have to second Godhand, even on Easy it's tough and the game can't even go to the highest difficulty. On Normal, it can but goes back down as you get your ass whipped. On Hard, the game is locked onto the hardest difficulty: DIE! And you will. A lot.

Ninja Gaiden series bt not 2 because it's much easier
Oh and I can't remember the name of it but there is a ps2 game which no one has ever beaten on the highest difficulty.
Not even the developers, have fun.

Heh. Towards the end can be murderous at higher difficulties. You get to play the "Outrun the monsters while frantically trying to repair your gun" game.

I fucking HATE monkeys because of that game.

You can also do Civ 3. On a higher difficulty (not even the highest) some random *barbarians* ran over my village driving M-1 Abrams while my people were still trying to figure out which side of the pointy stick goes in the enemy.

I have to agree with the people who are saying STALKER I got my ass raped repeatedly by a couple of shitty Conscripts early on just holding a checkpoint it took me like a thousand tries till I got around it. Of course the bad part is you sometimes get cocky like I went into this lab with all this whacky shit going on so I felt pretty badass after defeating the monsters in there and walking out but I wasn't paying much attention when the Spetsnaz team that had fast roped into the compound put a bullet through my head as I rounded the corner.
Oh yeah. And the Anomolies can kick your ass sometimes too. Many are easy to see, but one type is nearly invisible and all you get is a brief warning as your 1 foot away from setting it off, and then a wooshing sound as it activates, and then you die. Ok, you don't die right away; it tosses you around helplessly for a bit, THEN kills you. If you're walking slowly these can usually be avoided, but god help you if you happen to be running. Also, backing up/strafing in confined quarters can cause you to step in one and die, while NOT backing up/strafing can cause you to die anyway as some bullets get too friendly with your face.

Since people have been bringing it up, I'll throw out my comment about I Wanna Be The Guy: If you've played a lot of old school games you'll probably get a kick out of it, but it's really just cheap kill after cheap kill. The boss fights are very cool though, if you can get to a few of those I highly recommend you do them. Or see if you can find a few downloadable game saves at the bosses.

Otherwise I'd throw out Ikaruga as basically the all time classic. The challenge is doing it without using a continue. Unfortunately the game will eventually give you infinite continues, but you'll get a ton of satisfaction out of trying to beat it on one continue, I promise!

(It is hard to find though, tis for Gamecube)

X-com terror from the deep on ANY setting. That game is as rock hard as it is awesome.
