OMG when i read the title of the thread, i was thinking about this exact sequence (the first one anyway)! i feel as if my mind has been violated. so since i cant put this one now, i nominate the first time you meet the flood in halo. shut up, i was scared.
It's made me a little obsessed with Scarecrow, like, I really loved Heath Ledger's portrayal of The Joker in TDK (I know, how original am I) But never actaully watched batman begins, had no idea scarecrow was even in it, I'm a bit of a batman noob, but, I absolutely loved the scenes with scarecrow. When you played it, did you go into a room (for the first dream) and have scarecrows voice keep saying "get out" just getting louder and louder until he was like screaming, then go back through the same door you came in from and then have the bodybag scene?
Spoilers good sir!

Oh, BTW, did you like the 'glitch'.

Every level in Thief: Deadly shadows (Im too young to have played to previous iterations) only because the loading screens told me that there would be zombies (I fucking HATE zombies) so I was being as cautious as possible trying to make sure I never got caught with my pants down by a zombie and whenever I would hear an ambient sound that could have been a zombie I would panic and run as far away as I could before turning to see what it was this was even worse when it was a gaurd hitting me in the back of the head with a cudgel spooking me and not some old pie makers shutters in the breeze so heres how it went; I took some damage let loose a startled yell sprang up, ran as fast as I could and climbed the nearest wall, the gaurd persuing me all the way by the time I realized it was a gaurd and not a zombie I was the stuck on the wall avoiding thrown rocks until I found a window to climb through, my brother sitting next to me dubbed my escape tactic "The Crybaby Spiderman."

Well, most of mine deal with a character dieing, it unnerves me since I generally like the characters...Spoilers btw

During one of the cutscenes in Halo Reach, a main character get's sniped in the head and killed. I knew she would die at some point, but it was just so sudden. I had to turn the game off for a day or so.
Clive Barkers Jericho before the final boss, two main characters are killed and basically explode.
And as for a scary moment? Playing Resident Evil 4 and getting killed by a chainsaw villager in the first area. I was about...12, and it seemed so tense. Also, it was my first actual taste of video game violence...it was just so graphic lol

My bad ;P yeahh, unfortunately I'd already heard about it though so it wasn't as much of a shock than it should be

The second level of Vampire the Masquerade. Where you've got to go through that mansion. Holy fuckin shit. Here I was playing a nice fun vampire game and then you go and spoil it by making my bowels empty themselves all over the floor. On my second play through I had to turn the sound off and put my own music on.
This. I can remember being a little tense near the beginning, but the, uhh, 'Dryer' bit spooked the crap out of me and it went downhill from there.

Still, how has no one mentioned System Shock 2? I made the mistake of playing that in the dark with earphones on one night and I quite seriously had to call in sick to work the next day because I could not sleep. Damn, I still get chills from remembering that game.

The Asylum scene in Indigo Prophecy. I could not play that one.

The whole game can be freaking terrifying, but that one scene in particular was amazingly scary.
This. I was hyperventilating more than the freaking detective (who you had to stop from hyperventilating).

However, I also nominate the asylum sequence from Second Sight. I did it all wrong the first time, and ended up trapped in a room full of psychotic killers and guards that all wanted me dead. I've never hid in a ventilation duct for so long before.

Also, the sheer LONELINESS in Uru made the sequence where you found Kadish's corpse positively terrifying, even though the game treated it more as pure tragedy than anything remotely scary (it's hinted that he died there before you arrive, mournful but peaceful a cappella soprano, etc...). The loneliness made the uplifting scene that follow (if you're really good at finding secrets) where Kadish's tomb is empty EVEN WORSE, because at least a corpse was better then nothing!

Resident Evil 2 at the part where the licker jumps through the interrogation room window. Full volume + alone in the house = wringing urine from boxers.

Pretty much the first 3 hours of Dead Space.
In the first hour of Dead Space, when you flee from the first necromorph in the game into an elevator and just when you think you're safe

The final 6 minutes of the world is freaky as hell in Majora's Mask. Earthquakes, red sky, creepy music and that freaky moon. Probably wouldn't be as creepy if it didn't come out when I was 5.

Max Payne - Where you in a dream sequence comes back to your murdered family's house, open a random door and the whole game turns black and you only see Max and a path of blood you have to follow while you hear them cry in the background. Think it was hard cause you failed the mission if you walked away from the blood and it was hard to see it.. yea kinda creepy..

My first time through the Auroran caves in Fable III. I may sound like a pussy because of this, but I was playing in the complete dark, only in the light of my TV.

I'm afraid of the dark, very much so. The TV normally lights the whole room though, but then...
well, you know.

And with Walter's eyes, I thought they took them rather than blinded him. The whole section just preyed off of my fears.

It was more unnerving than actually scary, but I was extremely tense.
