(Fallout 3) The Dunwich building....nuff' said
This and when I first went to the Goodsprings graveyard in Fallout NV. The soft background voices of the dead whispering freaked me right out.

Most of Condemned Criminal Origins freaked me silly. And some scenes from the Fatal Frame series. So many scary crap in those games.

Dragon Age: origins

"First day, they come and catch everyone.
Second day, they beat us and eat some for meat.
Third day, the men are gnawed on again.
Fourth day, we wait and fear for our fate.
Fifth day, they return and it's another girl's turn.
Sixth day, her screams we hear in our dreams.
Seventh day, she grew as in her mouth they spew.
Eighth day, we hated as she is violated.
Ninth day, she grins and devours her kin.
Now she does feast, as she's become the beast."

That part always gives me the creeps....

In Fable 2, when you open one of the demon doors you take a walk down a nice snowy path, see a really comfy looking hut... the moment you walk into it, there's a loud SWOOSH noise and everything is suddenly twisted, dead and burned. There are also those weird cages with skeletons hanging out of them along the way out of the door.

I hate that part.
Oh yeah I remember that!

....creeped me the fuck out....

Amnesia in general. The entire game is unsettlling, save for the chase scenes which are just intense and horrifying.
"HOLY CRAP WHAT IS WRONG WITH IT'S FACE!?"

I'm just gonna go ahead and say the majority of Amnesia: The Dark Descent. Scariest game I ever did see.

I think few games really scare us, as in REALLY scare. Most of them have cheap scares and such.
But going back ontopic, i have 5 sequences/games etc.

1: Entire Silent Hill 1. I turned off the radio because it scares you even more when you hear that damn static going off...

2: Half Life 1. The first specimen experiment scene. i was like 13, and when everything went black, and suddently you're surrounded by Vortigaunts.... holy cr4p...

3: Penumbra (the second one). when the crazy naked guy chases you around.

4: Resident Evil 3: Biohazard. Rare jump scare, at some alley when a zombie jumps out from the hood of some car.

5: Metro 2033, the underground missile silo. It wasn't really scary, but it had a LOT of atmosphere. same goes for any STALKER game. Not scary at all, but beats any other game at atmosphere.

But seriously, Thief 3, Shalebridge Cradle holds a candle in my heart for scariness.
Me hating insane asylum's in the first place, and one thing leads to the next and suddenly, I need to do a load of washing

That segment in one of the Fallout 3 expansions (where you are on that island with the swamp and lighthouse) where you take some sort of hallucinogen and start seeing all sorts of crazy things. It made me very edgy.

Has to be X-18 or any underground section in Stalker. Bricks where shat.
Have you played Call of Pripyat yet? Jupiter Plant and the corresponding underground section made X-18 seem like child's play to me.

You see now im very scared, im on the first few missions of HL2 and people are telling me the ravenholm part is scary. And i dont do scary. i mean fuck the first zombie scared me.

just how bad is it exactly?
It depends. The zombies didn't scare me at all, so the section did very little for me, but seeing as they scare you, it might scare you, I'm not sure. I think that was my biggest problem with the game though, listening to everyone call it God's gift to the gaming world and believing it made the flaws all the more noticeable when I finally did play it.

Anyways, I have to go with the above mentioned Jupiter Plant. Dear God that place terrified me when I was playing it, though largely through my own stupidity.

Condemned 1
There's one part where you are following a trail of film developing fluid, you are using a UV handheld torch and everything else is pretty much dark as shit. You have been following this trail for a while, until it goes up to an edge of an elevator shaft or balcony or something, and some fking psycho jumps out over the rail. Skin was crawling after that.
Condemned 2
You are opening a row of lockers looking for stuff in that old school, near the swimming pool, and all of a sudden there's a 'corpse' in one of them. After you get over the shock of discovering him, he suddenly starts talking to you....yeah.
Quake IV
The part where you become a Strogg...it's a bit...violent. But the Strogg are disturbing in general anyways. Where you see what they do to people, it kind of gives you that nasty feeling that they can do anything to you, and they don't have any sense of humanity or pain or torture or anything they just cut and splice and saw and grind....yeesh (shudders)
Deadspace
When you board the other ship and see those infected humanoid guys twitching and moving ultra fast, worse when its just the upper torso...that really gave me a feeling of hopelessness. Protagonist Isaac is one tough mofo though, who never crumbled under pressure so that was a bit of a relief.

I don't know if anyone would remember this but all the way back in Timesplitters 3 with the zombie house level. When the deer head came to life and bust out the wall...a change of pants may have been in order.

And the bear chase in Condemned 2....*shudders*

I actually just bought Penumbra Collectors Edition. Being arachnophobia, the tunnel scenes scare the living fuck out of me. God, I hope there's no more spiders. Please tell me there's no more spiders!

This or any lab in STLAKER SoC or CoP especially near the end when you hear the baby. I almost cried.
But the SCARIEST moment was the water demon level in Amnesia. I literally froze in my chair, fingers and hands stuck to the keyboard/mouse.
