Eternal Darkness hands down. REmake was pretty damn scary I must say. Most scary game I have played recently has to be either FEAR (1 and 2) or Dead Space (The woman singing twinkle, twinkle *shudders*)

Doom 3 - When it was new.
Bioshock - Before I realized that I could bitchslap everything that came in sight.
Left 4 Dead - When Im the only one left and running for it.

But I have to say the game that scared me the most was.....

Diablo 1. I was a little kid and the first time I played that I was all alone in the house and i was playing it in the dark. Only game ever that gave me nightmares. Kudos Blizzard.

Penumbra - Black Plague

No game has ever terrified me so much in my life. I will openly admit that I jumped on more occasions than i can care to remember. If you havn't tried and you like scary games, you should really check it out.

When I was little it was Alone in the Dark. Those shadow things were creepy. Also you always ran out of Ammo.

F.E.A.R... I played that when pretty much anything would scare me... The demo even games me nightmares for a month... After playing that, nothing was really that scary anymore... Probably buying Dead Space today though...

I haven't played a new scary game in a looooong time. The original Resident Evil scared me when I first played it and I still think that it has a great scary atmosphere. The series however, has become progressively more action oriented as it has gone on.

I recently played Vampire the Masquerade: Bloodlines and I was actually surprised at how creepy the Ocean House mission was. I went so far as to say that I might have made a mistake by playing it at 3am.

The only real game I remember PLAYING that scared me was Zombies Ate My Neghbors...I was like 6 years old and my Grandma got it for my brother and me...It started off fine with the first two levels...But then the third level had those Chucky dolls...And the fourth level...the fourth level...That level alone makes me not want to play it...Yeah, Starts off with music that lets you know you will die...Then Jason with a chainsaw burst through the hedgemaze... The only way to kill him is with the monster potion or bazooka...Or you can DISTRACT him with clowns...So most of the time you are running away...NOT FUN AT ALL...And completely traumatizing the first time you try to win...Oh and most of the soundtrack is creepy as hell....

This is probably same for me. Especially when the teacher pops out at you when you take his picture. I almost literally crapped my trousers.

I simply can't tolerate scary games. I rented Bioshock a while back to see what all the fuss was about. Going into that dark lighthouse made me very nervous, and when I got in the bathosphere and arrived in Rapture, that splicer with the two sickles scared the crap out of me. That guy came in over the radio, and I was like, "You want me to go out THERE?! No, screw this game!" Ugh. Anyway, The Elder Scrolls III: Morrowind used to scare me too. It took me a long, long time to be able to go into the crypts and dungeons. Strangely, that didn't happen at all with Oblivion. In Fallout 3, however, I flee in terror of feral ghouls. Even with my Tesla armor and plasma rifle, I can't stand ghouls. I also get a little bit nervous around super mutants, but not at all around human enemies.

Jesus that was terrifying. It was the longest level of the game too.

Also, Penumbra: Overture and Black Plague. Overture got me with that one room in the writing on the walls (you know the one), and Black Plague with that god damned dog-man you could never see.

If mods count, then Afraid of Monsters for Half-Life 1. That mod was evil.

but chesty was your super duper special friend. Anyway, I feel like a pussy for saying it but bioshock made me jump alot.

Ya that whole level was weird especially when u jump into the drained pool.

Couldn't agree more, the department store level when the mannequins started teleporting towards you and duplicating, I was dying.
Ya thats the point at which the game started getting creepy for me. I found myself hitting every mannequin in sight to make sure it didnt come alive.

Lots of people have already said this but Fatal Frame was the scariest game I've ever played. It's the only one where I actually had to stop playing for a little bit because i was getting too worked up.

Definitely Silent Hill. I used to sit up alone, at night, in the dark, playing this game and it scared the crap out of me.
