Years ago playing EQ with my mid-level Druid and farming HQ Lion Skins in N.Karanas for quiver attempts... Luck had it that I ran across 3 lion spawns in quick succession, I rebuff my wolfform, and as I crest a hill near the zombie altar/crest I spot the rare Lioness that I need for the bloody skin drops.
(Gotta hate a 5% drop rate from a 5% chance spawn... for an item combine with a maximum success rate of 1-in-3. Has to be my number one reason for moving on from EQ.)

Much more recently I was making a medals run through MGS4 and was at the point where you must distract a Gecko from detecting Otacon's bot at a doorway. My 3rd attempt on this run, actually...
- First I forgot that you can't one-shot a Gecko with planted C4, no matter how big the pile may be.
- Then I forgot to reset my weapon loadout and plonked it with my .45 and causing an [ALERT].
- The third death was the real gem... After over a half-dozen times of watching it do the same thing, every playthrough (step to the corner of the walkway, pause and make a cursory scan of the area, then leap to just in front of the far tunnel entry to be patrolling), it pauses at the corner, bellowes a long, mournful cow-like bellow and leap over the rail to squish Snake under a single four-toed cybenetic foot, BEFORE spotting the little mech in the corner and raising the [ALERT].
