The other day I was playing SD in MW2 and I'm the last guy on the team, right before me is the bomb and a claymore facing it (I'm behind the claymore). Everyone on the other team is dead and I have SitRep so the claymore is flashing red. Anyway, I zone out for some reason and I'm sitting there trying to defuse the claymore instead of the bomb. Then it dawns on me, hey that's not the bomb, there's the bomb. I proceed to step forward towards the bomb, into the claymore I was just trying to defuse, and lose not only the round, but the game, for my team (everyone in the game spectating on my rediculous fail).

It was pretty bad.

Oh, so YOU'RE the number 2 P.A.B!
I...can neither confirm nor deny that statement as I have no freaking clue what a P.A.B. is.
If you go on Screwattack.com and look for a top ten list with the initials P A and B then you'll realize what I just said.

All this talk about people getting killed by falling items has reminded me of my own demise. Once while playing a level in star fox adventures, Peppy dropped a tank on me. "Ah crap, my tanks dead." "Your tanks down! I'm dropping you another one!" *SPLAT*

Being killed by my own passing stupidity (Falling into a pit or something)or really strange deaths like being killed with a smoke grenade from across the map (BF:BC2)

Oh and a story I wont let go of; friend of mine and me were playing CoD:Mw2 when it was still sort of new and I threw a care package, she spawned in behind me saw the red smoke walked up to it, crouched and asked "What's with this-"

*Thump*

This probably belongs in a "Most awesome wins" section, but for someone out there, this is surely his most embarrassing death, so I will tell the tale here.
I was playing with my buddy at a Halo 2 tournament. We both sucked, but there was free pizza and soda and we were going to have a lot of fun just playing the game as a team for once. In our last match we were placed against some guys who were just...gods. I felt less like I was playing a FPS against them and more like it was a survival horror. So they are 3 kills away from winning (40 was the win). We had 3 kills total. So one of the guys is rushing me from across the map with a plasma sword, and all I had were a rocket launcher, an assault rifle, and there was a pistol at my feet, and I'm backed into a corner. So I shot off the two rockets I had at him and he dodged them both. I picked up the pistol and switched to the assault rifle and sprayed fire at him. He managed to weave enough that not a lot of damage was done. Rifle empty I switched to the pistol and unloaded all the ammo into him. Still alive, and now his reticule is red. He pulls the trigger and lunges. In a stupidly desperate attempt to not die I try to jump, accidentally hitting the melee button. I had managed to do enough damage that the melee attack hits him while he is in mid lunge and kills him before he hit me. I picked up the plasma sword and screamed "BOO YA, *****! I'M THE KING OF T---" BANG! His partner snipped me. Oh well."

Red Faction: Gurrilla. Free Fire Zone, level 2 Armor. I liberated the place and killed every EDF soldier and annihilated every EDF vehicle. I decided to level start leveling everything, as well. On one building, I was standing WELL away from ground zero, when a tiny, brick-sized, paper-thin, piece of debris hit me in the head. I was at full health, and that ONE piece of debris killed me. Insane difficulty may have had something to do with it, but I doubt it. When giant roof sections fail to kill me, and a single, tiny piece of debris does, I think something's up...
I lost out on at least 750 salvage.

I was doing REALLY good in the first Mario game, jumping over this and that, only to be killed by a lone goomba near the end of the level.

That, or the time I stuck myself in Halo 3.
I do that in halo 3 more times than I'd like to admit. Stupid Bubble shields reflecting all those grenades.

Rabbite bite in Secret of Mana. Adorable and embarrasing.
Aww :) Don't worry I did too ¬¬

Uhh off the top of my head:

- Forgetting to purchase a certain item from the chemist in Leisure Suit Larry.
- Dropping a bomb and trapping myself countless times in Bomberman
- Jumping out of the hovercraft in HL2 and running myself over
- Falling down a well in Lotro (but hey I got a title for it!)
- Forgetting to equip any weapons/armor in *countless* JRPG's and running into a battle to wonder why i'm only hitting for 1
- Logging into my shaman on WoW to show my mate something forgetting i'm in a mob filled area and when I close the Inventory window I'm dead. He found it hilarious :)
- Dying to the first goomba in Super Mario (I think this is a gamer rite of passage tbh)

I think that'll do for now ;)

A small bit of soil.
Playing the Harvest Moon game Rune Factory, I was tilling some land in a cave and didn't keep an eye on my HP/energy.

At least I died doing what I loved. Hoeing the ground.

Picture this. Left 4 Dead 2.

Expert difficulty.
Exits safe room after leaving the hotel on the first campaign.
Fights to the CEDA tent. Horde.
Throws pipe bomb down the highway, out of site.
Finishes the Horde off still around me and waits for the precious boom.
Boom.
EDDY VAN ELITE HAS STARTLED THE WITCH!

And the rest, as they say, is history.

Battlefield: Bad Company 2 on Xbox360.

About to stab a dude in the back, suddenly I get the 'Please reconnect your controler' message, as it turns out, my battery died. And before I managed to turn it on again, he turns around, spots me and proceeds to run behind me and... well, that just sucked to be honest.

Haha I turned my sounds off while I was playing sc2 beta. My enemy sent a Scv to scout me and I saw it but they usually send it back after they scout. Anyway I was doing my build order and then I realized I was out of minerals. I saw that his scv was killing all my workers O_O needless to say I feel behind and lost. Never turned my sound off since then.

At BC2 we were getting baseraped by an apache (there were a total of 5 players on the server) 2 on my team including me and 3 on the enemy team. Well me and my teammate decided to go into our own helicopter but it were almost shot down after 5 secs so I bailed. My teammate survived so long so I could reach the ground and start running to the AA. The helicopter right infront of me, if I would'va been able to take one more step before the helicopter crashed I would've died. I just stared at the helicopter and said to myself "That was awesome!". Turns out the helicopter wasn't fully destroyed, it was burning. And what happens to burning vehicles in BF? Shit goes boom. I facepalmed as I had time to get away but didn't.

Can't really come up with anything exept that right now.

Throw grenade at wall, grenade bounces back, boom goes the idiot.
I do that at least once in every shooter, it's a tradition.
