After playing Condemned I think I have a few:

1: The "Tibbits Scene". My friend warned me about it and I still jumped. (He told me even though I hate spoilers in horror games)
2: The psycho who scratches his nails on the chalkboard in the elementary school. EDIT: (Shoot this mofo when you have the chance.)
3: In level (either 2 or 3) there's a stairway blocked by a few boards. I don't remember if you have to or not, but I looked down the stairway to see if I could jump down. Do not look down this stairway.

I know these moments seem stupid but when you're all focused on the game with the lights off things can get serious.

What does he represent? (message me if you dont want to post it here) I missed this game altogether for i was a wee lad (same as above) and there was no way in hell anyone in my family was going to get it for me. I need to go find a copy of it now that im thinking about it

Yeah, the SA-X was scary. I remember that one time, just before you get the Plasma Beam, where you actually have to confront it long before you're ready...still scares me..
Well, Dead Space's Hunter Necromorph was a character wasn't it? I mean, there was only one.

Anyway, for me, the scariest will always be her:
Don't laugh, that was my first horror game.

don't forget that that SOB has to be sneak attacked 2-3 times and you have to find the sharp objects to do it while WHILST avoiding him

I god how could I have forgotten, anytime I saw that damn thing my heart started racing and I died a little inside, and a lot in game.

WallMaster: Legend of Zelda series

This thing didn't make any noises, have any teeth, fangs, nothing. It's just a disembodied hand that silently drops down on you from the ceiling. Other times if you escape its fall-o-death it will glow green and fly right at you. Yet the worst is when you kill one and 3 more smaller WallMasters appear from it and if you don't kill them quickly they'll grow into full-size WallMasters.

Creepy disembodied monster hands.

First time I played Bioshock and that spider splicer killed that guy it freaked me right the fuck out, like "Holy fuck what is that." Since my TV has this bad habit of making shadows lightless voids of death and darkness so all I saw was the outline of cloth sack or whatever was on it's head and the glowing scythes.

Halfway through the game I was beating the ever loving shit out of them with my wrench, and somehow my static discharge managed to kill a few of them. But holy fuck seeing that for the first time not knowing what I was going to be fighting throughout the game especially after a highly immersive opening sequence.

Giygas... Jesus fuck... that and the music played during his stage as well... ugh, still unnerves me.

Second place I would say Silent Hill. The town itself is a character
