An actual city. Most the "city" parts of DC were roadblocked off, so it was linear. Same with city with the death claws. New Vegas was just a joke.
If you are going to have a karma system (and other systems like crafting in NV), make them actually do something. Karma in NV was completely meaningless and in F3 is changes your companion options and which people hunted you (so a clothing change basically)...
Which would undermine the choices you made throughout the game, and what exactly are they supposed to do with the post-game map? Pretend that the battle for the Dam never happened? A setting in Europe would not only remove what made Fallout Fallout but also would break the lore. Europe and most of the rest of the world is gone, it was bombed to hell, like three or so times. (Not talking one bomb drop, but many bombs dropped each time.)
