For me it would be Fallout 3. Except for the main menu music at about 30 seconds in. Of course, the start of that is rather annoying. But the idle, "wandering" music is just so damn annoying.
Funny. I would completley disagree. I thought the music in
Fallout 3 was very good and great for creating immersion. Though I guess this is why music is considered a very subjective thing.
Anyways, I'm actually struggling to think of a game in which I really thought the music was terrible. This is because for most games I can pick out at least a few songs I thought were decent so I hesitate to say their music was bad. Otherwise, bad music in games is just forgettable so I can't actually remember any terrible music. So I'm going to have to say
S.T.S.L.K.E.R: Clear Sky. The music wasn't bad, in fact I thought there were some very good immersive songs in it. But this installment had some stupid "fight" music that would play whenever you got into prolonged gunfights. This had no place in the
S.T.A.L.K.E.R games and really didn't go along with the game general feel. Because they, in my opinion, worked against the games immersion, I have to say that they are bad examples of video game music.

Yoshi's Island DS.
Compared to the absolute masterpiece that is Super Mario World 2: Yoshi's Island, the music is complete garbage.

See this is an example of a game with just forgettable music. It's not necessarily bad, just really dull. Which is, like you said, pretty bad when you compare it to the original game. Though personally I think these two songs are the best example of the difference:
You can tell they didn't put much effort into the music in the DS game.
