I had a form of habitytis(or however its spelt) in my liver. At one point I was on the verge of stabbing a steak knife into my heart.

I had two teeth crack open and the exposed nerves throbbed with pain for two days before I got to the dentist. 48 hours of writhing around in bed not eating, drinking or sleeping.

The pain was so intense it was the single day of work I've ever missed in 6 years.

Damn, that's a tough one...I can't pick between a couple, so I'll just list 'em.

When I was 8 I was riding my bike down a gravel road and lost control, jamming the handle bar into my chest, dunno how I got home but for the next six hours I was rolling around on my living room floor trying to get a sustainable amount of oxygen into my system.

Or recently when I was hunting I hopped a fence and landed in mud, causing my foot to slip out from under me and landing on it sideways. The pain was unbearable and I had to walk across two miles of cobbled fields and climb down and out of a 20 foot ravine to get back to our staging area.

Oddly enough the sprained ankle hurt a heck of a lot more than skinning my knee down to bone on gravel and having to pry out bits of rock out of the flesh for 2 weeks later....

Colon cramp. I have NO idea what it actually means but that's what the doctor diagnosed me with.
It started around 12:00, just slight pain in my lower belly. I slept for few hours and woke up, the pain was getting worse. I have a sensitive stomach, so regular painkillers would have only made it worse. By 18:00 it was hurting so bad that I couldn't stay still, I was pacing back and forth around the house. Then I sat in the toilet for few hours, puking and sh*tting my guts out, I puked so hard that I got a nosebleed. The pain was just getting worse, by 21:00 I felt like I was going to go mad.
By 22:00 my mother finally agreed to take me to the hospital(she thought it to be just extraordinary nasty heartburn). 30 min drive there, and 15 min waiting, I was literally writhing in pain. The nurse asked me bunch of stuff, I gave vague answers, "yes, "no", "painkillers plz". I'd liked to be more cooperating, but it just hurt so much that I was ready to start sharing the pain with others.
Then FINALLY the doctor took me in, poked my belly a little, said that my pee- and bloodtests hadn't shown anything fishy, so he suspected a colon cramp. Injected some sweet relief straing in my bum and sent me home, thus ending my 10 hours of pain.

The runner up is when an atheroma(http://en.wikipedia.org/wiki/Atheroma) was cut from my scalp, twice. Both times there was a nasty infection on it, so the anesthesia didn't kick in as much as I'd had liked to.

I crushed two vertebrae in my spine when I was 9 and then tried to stand up afterwards...

And then when I got to the doctors she made me try to stand again...

People tried to tell me that breaking a leg is worse... it's really not. A lot of people don't understand how painful spinal injury can be, as damage to the spinal cord will stop the pain from getting through. In my case, I was both very lucky and kind of unfortunate as my spinal cord was ok, and I could feel everything.
