I could be playing any game when my headphones crackle and it'll send a shiver down my spine every time.

Call of Duty 4 is the godfather of all the unpleasent sounds in the gaming industry. The "click" of a claymore, when you have a second to live, the "tink-tink" of a grenade and the sound of an approaching enemy helicopter or an airstrike.
Oh, and that creaking sound in F.E.A.R. when you know something is going to happen. I usually just blindly run forward.

ANYWAY on topic: For me it's delayed sound I don't know if I'm the only one who gets it but it's annoying... *moving mouth* two seconds later "hello" one second later *me hitting tv or sound of annoyance*
