Of the games that I have right now? Whenever the "tank" music starts in Left 4 Dead, especially near the end or in a tight corridor (which the tank can impossibly fit into). Damn it I hate that sound.

A close runner up would be the "!" sound in any of the Metal Gear Solid games, when a guard sees you. Damn I hate that sound.

Any kind of sound that is followed by a massive attack.

From the click of a mine/grenade, incoming weapons fire (sniper or rocket), to warcrys of badguys (the flood in halo 2 for example).

I got the fright of my life when I heard a bird make the same sound of a incoming fireball attack from flyer in the game 'hostile waters'. For people who have not played the game, these flying mosters would just apear out of noware. Best way to describe them, is a flying zergling with a ranged attack.

Needless to say nobody understood why I had ducked for cover.

Oh, I got another one. That terribly annoying sound when you miss a note in Guitar Hero. Ugh! It's even worse if your fingers are on the wrong buttons and you screw up several notes in a row. Sounds so bad in the middle of a cool song!
(I'd be surprised if this one hasn't been mentioned before but I'm not digging through twelve pages to check.)

Only sound in a game that's ever scared me was playing the original Medal of Honour.. When you're crawling around the air vents.. and you hear a "tick.. tick.. tick.." of a German with an MP40 crawling around too, and you don't know which end he's going to arrive, and if you guess wrong you get an ass-full of the Omni-gun SMG rounds that all SMGs can use... ARGH!!!

Sorry.

I did love that game, and MOH: Underground. Probably because they were the first FPS's I'd ever played outside of an arcade, and had nothing to compare it to.. And they had a halfway decent story, unlike the newer MOH installments.

The "Keee!" Style noise oblivion Skeletons make. By the Nine, that GRATES. Particularly when you keep hitting them, and they don't stop making it until they die. Then they make a looooong version, as they die. Then you fight the next one... And the next... And some more... And the Necromancer thought it'd be a good idea to summon More Skeletons.

You get the idea? I have to turn the damn speakers off every time I fight a damn skeleton.

I will have to go with:
The death scream in any of the "Kings Field" games.

When you hear it, you know you are dead and you know you've been playing for hours without a save, and you know you've now just lost all of that progress.

either on cod4 when u run round a corner and hear *click* or on Counter strike - source if u get flashed and u can hear the ring after wards but kinda glitches and dsnt stop.
