When I saw the T rex for the first time in trespasser.
First I was like holy shit when it lunged towards me but I dodged it by crouching and then ran like hell from there.
After that I was like "ohshitohshitohshitohshit" when I turned around to catch the T rex running like a horse behind me.
For a crap and buggy game, that was quite a moment.

Operation Flashpoint Resistance.

I was playing Serial Killer online. In that gamemode, 2-5 players play as serial killers. Their mission is to kill civilians all around the island. Rest of the players are cops, who try to arrest or kill the killers.

It was one of my first times as serial killer. There were 3 of us: me, one friend of mine and some other guy. Me and my friend teamed up and the 3rd one went alone. Two of us decided to take shelter in some apartment buildings. There were two of those buildings, so I went to the other one. We had sniper rifles, so we started picking up those innocent civilians. After about 3 minutes we heard a helicopter.That helicopter did a kamikaze attack on the building my friend was in. Needless to say, he died. I thought it wasn't safe to stay inside, so I ran outside. And, one minute later, another helicopter kamikazed the building. Lucky me.

But it didn't end there. I took shelter in a smaller building. I saw policecars and some armored personel carries, so I decided to stay quiet. After few minutes I looked out of the window. What did I see? The bad end of a tank barrel. That tank was sitting there, waiting for me to stand up. Many bricks were shat.

Mostly because it didn't fuck around the issue, like other games. And just killed him stone dead.

Okay, so I've been fucking with these guys for some time, raiding their convoy twice (Merc Alley, and Merc Alley A respectively) and taking some pretty sweet stuff. Well, rule of three dictates that I do it again. It also dictates that something big is gonna happen.
Me and my lance arrive on the scene, and the Voice with an Internet Connection lady tells me she can't get me any info on what I'm facing, 'cause IT'S A TRAP they are jamming her whatever fancy-schmancy radar she has. So I go over the ridge (the place is in the desert), and look for the convoy. Nothing. But I detect a bunch of demolishers (anti-mech tanks) and some other military vehicles. Then an Argus. Then a Catapult. Another Catapult. And then about 10 more fucking 'mechs from behind my lance. Holy crap indeed.

For them. I thrashed all of those motherfuckers without losing a single 'mech. Definitely fun ^^

Your first encounter with a Tank EVER on any Left 4 Dead game. All normal Infected, maybe a couple of Hunters here and there, then suddenly BOOM! Car flies at you.

The 'ending' of Red Dead Redemption. Nearly made me cry.

The ending of Super Mario Galaxy. That actually shocked me, didn't expect Nintendo to go down that route.

Ghillies in the Mist from CoD4. All of it. Was just spectacular. Hasn't been matched in any later games. Closest they've come to matching it was in Ciffhanger in MW2, but it wasn't in Chernobyl, which makes it less awesome.

When Desmond is frozen by the apple, and stands there till you hit a button, and he starts moving forwards. Then you realize his hidden blade is extended. And you try to make him move back, and you can't...

For me, the T Rex on the original Tomb Raider scared the fuck out of me, particularly as the music always indicates incoming trouble. When I first saw the red dinosaurs, from a distance I honestly thought they were foxes. Then they got bigger, I heard what sounded like someone running in wellington boots, and ran for the ledge at the start.

EDIT: sorry, didn't really read the OP before commenting.

erm...the beginning of Dead Space 2 held one big 'Holy Crap' moment for me. having to run through corridors with Necromorphs bursting out of the fucking walls and ceilings and I could hear them chasing me but didn't dare to turn the camera round or stop to look cause I knew that if I did they'd kill me! holy shit was that a fucking tense gameplay moment!

also: Amnesia. I got my hands on it at my friend's place the other day...was quietly saying "oh holy shit..." to myself throughout the entire time I played, then when I saw a monster it quickly turned into "HOLY SHIT! HOLY SHIT! RUN RUN RUN!"

The first time I saw the Valkof. (from 1:09 to 1:30 or so, my jaw was dropped) Knowing that I had to fight that thing afterwards didn't really help...

Also, fighting Nyx Avatar in Persona 3. Hardest two hour long battle I've ever faced... (in case you're wondering why it took me two hours, allow me to say "no Persona fusions, and I fought him at lvl 77 with squad-mates at 76-77")

Oh, and as many others have said, the nuke scene in CoD4:MW.
