COD WAW: on the hardest difficulty: Revealing one square inch of your body results in some every opposing person on the other side instantly unloading every bullet on their person at you, completely ignoring the other soldiers who can somehow stand in the middle of the battlefield and not get shot.
Not letting them see you results in them throwing their entire grenade supply at you, and only you.

N64 Super Smash Brothers: Whenever I play with my friend (we like to set it to 200% damage mode), knowing I will pick Kirby, he picks Mario and either grabs/throws me to death, or uses his up special to jettison off the map no matter what damage percentage I happen to be at.

Left 4 Dead Witches.

Their knock-out one hit (or knock out dead hit if your a poor bastard playing expert), is so annoying, espsecially if your night plans didn't include getting ripped apart by an ugly girl in a bikini.

I hate when the director drops them somewhere completely unavoidable too. I've had them appear at the top of the ladder to the roof at the end of No Mercy: can't go around her and can't shoot her easily. Can't even fire from the ladder... cheeeeesy.

I know. Once, my team was doing so good on No Mercy Versus that it gave us TWO unavoidable witches. My team was not amused. Oh, by the way, the solution to your problem is the same solution to enemy Hunters camping the hole: throw a molotov up, the entire roof will be on fire.

EDIT:Oh, you know what's even worse? When there's an avoidable witch, but the game sends hordes at you so you never hear her crying and stumble onto her. It's worse because you're like "Oh, I could have avoided that! I'm so stupid!"

The final boss in SSBB's Boss Rush mode. He has an instant-kill unblockable seemingly undodgeable(I'm not willing to commit to that point, I'd like to think they give you some option, even if I've yet to pull it off.) attack that hits you regaurdless of where you are onscreen. That's some pretty serious BS.

I was gonna say Theseus' ice pillars in God of War 2 on Titan mode, but at least I finally beat him, and I didn't have to fight through a dozen bosses between each try, either.

Persona 3. I forgot the boss, but I was slowly beating this boss that kept using Wind spells. I had a lot of wind resistance which made it pretty easy. Then out of nowhere he pulls out this extremly powerful Fire spell and kills the entire party, most near full health. I gave up on trying to beat him for awhile after that.

In Call of Duty:World at War when your friends who, by the way are all level 60, invite you to play a nice "friendly" team deathmatch, all join the red team and "work together" to kill the level 7 player. Yeah, that's real fair.

Yeah. Ugh....I remember the first time I realized that sniper rifles didn't work against them...ugh. Well the ones in Halo 1 with the rockets were pretty annoying. They killed you very fast. Especially on Legendary.
