I was on the front of a bike as a kid and put my foot in the moving wheel. I shot over the handle bars and got run over by the bike. I broke my left leg in two places and fractured my right arm.
Also, on my 18th, I fell over with a pint glass in my hand and cut my arm. I found it quite amusing when I was licking my bone... anyway, I have a scar on my arm in the shape of half a pint glass.

I managed to stab myself in the right eye with a thumb. Don't ask how, even I don't know.
Almost lost my eyesight in that eye and had to get all kinds of medication for it for about two months. xD
Edit: Oh, and there was that one time I was riding my bike, and there was a tree (not just a branch, the tree itself) that was sticking out onto the street at about my head's height. I hit it while going really fast, it knocked me off the bike. I almost passed out and it hurt like... well, being hit in the head with a really heavy blunt object.

Edit: Oh, and there was that one time I was riding my bike, and there was a tree (not just a branch, the tree itself) that was sticking out onto the street at about my head's height. Trying to do a dance move (the reject, google it) on concrete in football boots, 6 weeks in crutches.

Walked into a lamp-post and cut myself cleaning a knife.

first (and last) time i tried inhaling nitrous oxide out the back of my work.
loaded 3 cream bombs, breathed it in and to spin myself out decided to do a little roll on the ground.
All that was fine but as i got to my feet after the roll for some reason i dived straight into the brick wall in front of me knocking myself out. As i came to i was face down on the cement banging my face into the ground having a fit. My mates picked me up and sat me down in a chair.
I was laughing at this stage but then i moved my tongue around my mouth. finding a large gap where my 2 top front teeth used to be. They were embedded in my bottom lip.
This was at about 11:00pm so we had to call out for an emergency dentist to come into town and open up his clinic. When he got the sat down and had a look he said he'd have to remove the broken bits of tooth left in my gum and asked "would you like some Nitrous for the pain?" I just laughed and asked for a shot instead

Sailing down the stairs in my ridged toy box lid which made me do a flip and land painfully. I was 7 and kinda want to try it again to see if I can master it.

One of my friends skinned his knuckles when he did an offhand hook at another one of my friends. Except the wall got in the way, and it wasnt a smooth wall AT ALL.
I once nearly cut off my finger, and had the same. No pain, no blood on the knife, hell, my finger was barely bleeding, and the only thing holding it together was my fingernail. I was too ashamed to tell my parents, soo I walked around with my hand in a fist for a full day, until they discovered bloodspots on the floor.e2density said:
I remember riding into a guide post once on my bike and it broke the reflector light thingy on it. Not sure how I did that.
@offtopic: I also managed to shatter a glass while trying to remove it from another one with my bare hand. I don't know how I did that, and I don't know how there wasnt a single scratch on my hand. @_@
