Either be extremely homophobic, racist or some other kind of prejudice, extensively verbally abuse another member or (especially) a mod. Also you could get the "Straw that broke the camels back" scenario in which the mods just get tired of giving you endless probations/suspensions and decide to perma-ban you (a la MaxTheReaper).

On topic: Pretty much everything mentioned above, being overly agressive and spamming, posting unsuitable material, making the forum a not so pleasant place, etc. This post, granted, was completely pointless as all it did was repeat that which has been established but it was done with no real malice, plus it could be interpretted as constructive were it not for prior posts, so it's OK... I hope...

1. How easy is it to get permabanned? Very, depending on what you do. Spambots, advertisers, ban-jumpers, porn posters, and the like get a first class ticket to Banville as soon as we spot them. For other infractions - say, your typical troll - you generally need to show a pattern of behavior. Most folks are capable of losing their temper and doing something stupid once or twice, and while they'll get slapped for it, they typically won't get permabanned.
2. How do you get unbanned (once you've been permabanned)? You don't. Permaban means we don't want you here. You've broken our rules to the extent that you are no longer welcome in our community. Period, the end. Other bans may be overturned by appeal (i.e., you basically explain your side of the story, and if we see your point, we lift the ban), or you can simply just wait them out. But once you've been shown the door, we really have no interest in letting you back in.
And Max didn't ask to be banned, he simply was banned. He did, however, thank us for it.

also, I looked at the "user was banned for x"
and it was a post about how he wanted to quit the escapist with the words at the bottom
"user was banned at user's request"
or am I thinking of a different person? he was the one who ranted about hating iphones right?
also, about how easy it is to be banned, just post anything that somebody who frequents /b/ would recognise easily
