The mole rats in Fallout 3 sometimes make me jump and curse. You never hear them from behind, luckily my shotgun is always primed to blow their heads into tiny pieces.
I was playing through Half-Life 1 awhile back and I almost fell out of my chair when a barnacle that I didn't see starts sucking me up after I thought I had cleared the room of the buggers.
Playing Borderlands, and just up the bit in the Arid hills, after the big skag fight, I was aiming at the one of the bandit snipers with a sniper of my own. I aim for the head, shoot, but there's an invisible wall.
Rats.
I move over to the side, aim again, and shoot.
*Tink*

Thief: Deadly Shadows. Down in that ship. There are zombies who lie down on the quite zombie-coloured floor and leap at you when you get too close. I hadn't seen one of them and it flew up as I ran across the room after some loot. I literally jumped in my seat, and my heart kept pounding for ages.

L4D's tanks are pretty good at shocking me, even if I can hear them I just get shocked when they sprint twenty feet and punch me square in the face...
My brother was playing Resident Evil 4, and thought he had killed all the enemies in the area. As he walks by, one of them, that has at this point already had it's head removed, forgets that it's dead, gets up off camera, and grabs him from behind. Now, I was sitting next to him, reading, and therefore did not see what caused him to release a scream that chilled me to the depths of my soul, and very nearly stopped my heart.
Once in Fallout 3 I looked down from a scaffold and saw an ant queen. I thought it was a Ant guardian or something jumping too attack me, and I have this phobia for insects (when they get too close). Anyway I jumped through the celing and landed on the roof. It was raining outside, not fun...

Oh and also I've been surprised buy the god-damn Mole Rats more times than i can't remeber.

oh and the Spider Splicers in Bioshock. you can hear them, but then they rip your guts out with their hooks.

Fallout 3, So I was walking through the wasteland minding my own business then bam! 200 rads per second then I fell into a cravats where the rads was 600 I used up all my Radaway yelling No!!! No!!!!! then I died and had to start all the was from Bigtown again...

this one time in l4d when i was abit new a boomer suddenly appeared right infront of me while i was shooting. i dont mean it came around the corner or like this one time when one nearly fell on me, i mean i was out in the open and their was nothing for it to hide behind.
