My last Mass Effect playthrough when Ashley killed Wrex on Virmire, due to my Charm being too low. I knew that you could get through that without losing anyone, but it was Urdnot Wrex, the most bad-ass Krogan ever.

When Shepard shot Roach and Ghost. Unfortunately, I had the 'Just Like Old Times' mission plot spoiled for me prior to playing, but I didn't know why Shepard became a bad guy. Then he kills two 141 guys and burns them.

In Fallout 3 when James died, I actually didn't really believe it until I finished the story. Then I got all sad.

Crono's death in Chrono Trigger - Talk about a plot twist. Sure, you could bring him back, but while this scene was happening a lot of people didn't know about that. It was mindblowing, to say the least.
Andrew Ryan - While I was playing Bioshock, I was PISSED at Ryan for killing Atlas's family. The whole game I was like "I'M GONNA KILL THAT BASTARD D:<". Then I got to the confrontation, expecting it to be one hell of a huge fight. But something wasn't right. I was like "Ok, the game is going to throw me a curveball..." Then I walked into the room... and well, you know how it goes. I suddenly started thinking "Ok, I don't want to fi- wait, what? Hey, WTF? Why is Ryan asking me to kill him? *smack* WHAT?! *smack* NO YOU IDIOT! DON'T KILL HIM! DO- *smack* *dead*." My reaction: "O____O" Then I found out that Atlas's family didn't exist. Talk about a kick in the balls.

The PC's death in Jade Empire, and the whole Kuei-Jin ending in VtM: Bloodlines. Both of those surprised me so much

I mean, YOU die in MW. I never expected to die by nuke.
And MW2 I was kinda seeing it coming but when Ghost and Roach died it caught me off-guard :/

Aeris, Final Fantasy VII... so sad.
There was supposedly a way you could bring her back to life, I tried for days and days, it really was horrible.

I agree, the death of Aerith was the first time death hit the player in such a fashion.

I also remember that rumor about being able to get her back, but it was just a rumor. I played that game for at least 500 hours (not exaggerating sadly) and know more about it than any human has a right too.

Pvt. Jackson from Call of Duty 4. You're riding a victory wave, having rescued that downed heli pilot and are hauling ass away from the city, only to be nuked. At which point you have to stumble around for a minute, horrified with what's just happened and hearing children laughing. And the worst thing is, if you hadn't rescued that pilot, you probably would've made it out of there.

I agree and I wish they had done that bit differently there should been certain scenarios like one where you made it out fine one where the above happens and one where you get the pilot but your too late and the nuke goes off before you get back to helicopter depending on how much time it takes you to get her.
