It is very hard to kill a vigilant, defending person with a knife. As part of my law schooling, I work at the prosecutor's office and have an opportunity to read dozens of police reports along with other court documents. Most knife attacks are non-fatal, and even then the victim receives as many as 20-40 cuts and puncture wounds. And these are on civilians - imagine against a trained soldier.
I'm probably the only one who cares... but I would propose "grapple-style" combat when someone tries to melee an aware target, similar to what Call of Duty 3 had, but not as scripted like in that game.

The problem is that if the knife is not a one hit kill then it would be more useless than the plasmids in Bioshock. The knife has always been a weapon that while useful in its own way, is one of desperation (except with commando in MW2 which is why that perk no longer exists) to be used in a hectic firefight where you just used up all you rounds and someone is still shooting you, or at extremely close range. If the knife was made a two hit kill and I was in a situation where I had an empty light machine gun at point blank range facing an enemy with a fully loaded shotgun, I would have a better chance of survival attempting to reload my weapon and praying to god his reflexes are so slow as to not notice me for ten full seconds. Incidentally, you'd probably like BC2 since the knife really is useless in that game. Most of the COD Youtube community is crying out for the knife to be changed to a 2-hit kill. Nothing makes me angrier than when I outplay someone, get to the side of them and start to shoot them down, only for them to lunge through my bullets and one hit kill me with the auto-lock knife.
It's worse than ever in Black Ops, you don't even need Commando.

OT: Well, I always had the idea of knives putting enemies in a 'dazed' state, which they recover from in a bit longer than it would take otherwise. That way, knives wouldn't be a one hit, and you'd get a chance to shoot or knife your opponent back, but it'd still be a powerful reward for getting close.
