Lots. The Meat Circus in Psychonauts, every damn stealth section in Bully, The Fade in Dragon Age: Origins, the Tokyo level in TimeSplitters 2 and The Deal mission in GTA IV.

Resident Evil 5: The deadly light mirror puzzle, especially when combined with a co-op partner who thinks it's oh so funny to light you on fire and start the whole thing over again.
Dead Space: The ADS cannon. It's not scary, it just makes me despise Hammond for saying "give me a minute" and actually taking a full 60 seconds while I miserably attempt to hold out for 30.

Well it's pretty hard to see with all that money in front of them.

OT: All the parts in Stars wars Battle front when I'm not shooting anything. And the parts on the space levels when i'm not in a ship!

Scanning, hacking and bypassing in mass effect 2, I usually exit all hacks that tell me they have credits in. I also like do do a ton of scanning at the start of the game instead of doing it when I get the upgrades.

100 coin shines and blue coins in super mario sunshine, GELATO BEACH I AM TALKING TO YOU

Fighting polygon team on super mash bros. annoys me that I die at least once on very hard on it almost every time. I don't think I will ever get a no miss clear on that difficulty on that game.

HM slaves in pokemon, pretty much everything except fly and surf were weak

Spiny blue shells on mario kart... just... WHY!? In fact, it's the #1 reason time trials are my favourite mode on it.

PvP RNG in WoW, I rage in arena when my traps get resisted on my hunter and spell pen does nothing to help

Eternal engine in sonic adventure 2 battle. Specifically those damn blue alien blob things that I forgot the name of. Also the input glitch on 50hz where whenever I press A once the game registers it twice and I homing attack into nothing and die instead of doing a full jump or hitting the next enemy over that bottomless pit.

There are probably more, I might edit this later (I probably won't)

Water levels. Areas in games where the makers give you worse controllers and make it easier to die in. All at a slower pace.

Sonic games in particular annoyed me other then 3 and Knuckles.

Dragon Age: Origins: That fucking Fade level. Though shapeshifting is kinda neat.

Left 4 Dead 1 & 2: Any part featuring a Zerg Rush that forces you to run to the nearest safe place. Which is always, always at the end of a long hallway.

Mass Effect 2: Mining. And Jacob's and Zaeed's loyalty missions. Dunno why.

Gonna jump on the "We hate the Fade" bandwagon. Bloody hell, that was such a stupid level. It's not like the damn tower wasn't long enough as is...

Other than that, the entire end part of Vampire The Masquerade: Bloodlines (long title, eh). Basically, if your character was set up for anything other than gunfights in closed quarters, you were screwed.

There was a part in Modern Warfare 2 campaign where you are on the oil rig and they start popping smoke for cover. I must've tried to get to the damn breach door for 2-3 hours I was so pissed, especially when the campaign should only take like 8 hours to beat.

So then I played it again on the hardest difficulty and beat it on the first try...*kills self*

The Cobweb Duster in Psychonauts. Basically, how it was goddamn expensive, became necessary to progress right in the middle of one of the game's best levels, and how there was no forewarning that you'd need it until you got to that level. So basically, you have to leave, go back to the starting area, buy a device to look for arrowheads (the currency) and spend half an hour digging the things up. It's pointless, it's terribly positioned, and it's just not necessary at all.

Why is it that everyone complains about the Meat Circus, but nobody even mentions this?

Honestly, Mile High Club was a pleasure compared to Heat and No Fighting in the War Room. Oh, but I miss those levels. I might re-do the COD4 story.

Neo Tokyo in Timesplitters 2. I'm a perfectionist when it comes to stealth in games so it took me a helluva lot of tries to figure out all the patterns to a tee and get through it with little-to-no gunfights.
