I either name them after myself, a friend, or just look up a random name online. Except on one playthrough on Mass Effect, where I named my Shepard "Commander". Makes it seem like everyone is actually using his first name in conversation...

If I'm being boring, I use my real name or 'Knusper'. I like to make a mix of names of those that fit the game's world and those that don't, just for the laughs; for example, I called my latest KoToR character 'Captain Punctual'.

Most of the times i use my name (Michael), my late cat's name (Yuri), my dog's name (Hoshi), though sometimes i will also use names from random characters from Fire Emblem.

I usually go with a generic name like Fred or something, but when I feel like it I just go for a weird one. For example: Lasdriniflirastiuskrinisdalfrindian. Yes, I can pronounce that.

I started out using my own name, and the names of friends and family when given the option to name other characters (namely, old Final Fantasy games). That was pretty uninspired of me but, I was a kid.
Then after realising that game conversations were quite difficult if you couldn't even remember the main character's true name (Tidus in FFX) I have almost always kept the predetermined names given.

Although with Pokemon I'll always name the rival character something bad, hoho I remember having battles against "Poo" or "Willy" when I played pokemon Red and Blue at the age of 6! Haha... Serves you right!

If it's worth anything I named my dog in Fable 3 Dicksmasher.

...cmon, who won't be scared by that?

Usually I go with my own name, but that's mostly because mine is pretty original and different enough to keep the master of all whateversville I'm playing in have a bland and boring name, while giving me a better chance to project.

Setting you name as Revan in KotOR 1 is hilarious;
"I'm not Revan anymore, I'm Revan now!"

Other than that my standard names include Vokuts, Shassui, Ivanova, Amalthea, Leda or Susan.

The few times I actually make a male character it's Stukov.

Or if I'm in the mood I go for a punny name, like my most recent Fallout 3 character, "Psycho *****"
-Dad: "That's a pretty name don't you think? Fits you perfectly."
Yeah thanks Dad...

I usually make up a name that sounds fitting for the RPG, so if its a sci fi game, something with a more futuristic bent, and for fantasy rpgs, something with a "ye olde english" sound to it

Answer: The main character, unless it's memorable like chrono or link, gets their name replaced because, I'm sorry to say, half the time the main character has a very bad name.

First character I make is usually my name (even last name, if allowed, which actually fit SMT: Strange Journey). Second one and onward are the fantasy names. Examples of names currently in use: Cailus, Odorn, Ghikan, Muukran, and Rio

Why the hell is the thread called 'Titty Shepard'? o_O?
And on-topic, I usually have a range of names available, for both males and females, depending on my mood.
I'd imagine because that's what the OP names his Shepard.

I had two possible outcomes when seeing "Titty Shepard"
1) A person who shepards herds of wild breasts
2) A female Shepard from mass effect who is unnaturally well endowed

I was a little disappointed.

OT: I generally name characters "Hydros" but it doesn't really fit many settings so I'm thinking of new names to use. In games where the character already has a name I'll leave as is most of the time, but in pokemon I will always name the rival something stupid like newfag or some such thing to make it more amusing.
The name comes from ZP's review of Mass Effect. Thought it might help people understand what I was asking them.

I usually give them a name that reflects their personality, or physiology. For instance, in the fallout universe, I gave my character a distinguished name, but changed it when he left the vault to "Blob" because he was litterally a constitution/STR tank with almost no intelligence. I also use the name Shilleka if the character is kinda nature-liking, ranger/druid kinda personality.

I have plenty of other names I use associated with different personalities and such, and sometimes I just make up names. Only time I use the classic name is on the first playthrough.
