I lost Legion on my playthrough. He was my Tech Expert I sent through the ducts. I don't understand what I did wrong, actually. Everybody else survived. It was during the cutscene where the giant doors were closing, and the rest of the team rushed through. Legion went to help push the door closed and took a rocket to the chest.

To clarify, I had Zaeed as my Fire Team captain. Like I said, everyone besides Legion survived.

I've completed the game 3 times.

First runthrough, ME1 import of my Soldier save. No casualties.
Second runthrough, ME1 import of my Soldier save, re-adjusted to Vanguard. No casualties.
Third runthrough, ME1 import of my Soldier save, Insanity difficulty. No casualties.

First time through, Zaeed, Mordin and Miranda.

Zaeed because I incorrectly assumed that a veteran mercenary would be capable of leading a small squadron.
Miranda because I sided with Jack and never got enough points to regain her loyalty.
Mordin because I didn't have the sense to send him back to the ship with the hostages.

Every other run, nobody died.

Grunt but I didnt care cos Wrex was far superior. All other playthroughs everyone survived.

Im thinking of making a troll playthrough where almost everyone dies (except Shepard, and 2 others)

No one died my first time. If you import a Shepard and just...do everything, then no one should die. Well, and you have to make the right choices on who leads the team. The Squad leaders and tech/biotic specialists are obvious. However, the game doesn't let you know to leave behind your strongest defenders (Garrus, Grunt, Zaeed. Don't take more than one) for the final part of the Suicide Mission. Taking more than one defender and leaving too many weak defenders (Tali, Jack, Mordin) makes it possible for one of the people in the latter group to die.

Anyway, on my two main characters no one died. But I've experimented in other times and had some die. Tali died last time while escorting people back to the Normandy. I failed to gain her loyalty because I wanted to try exposing her father. She didn't like that.

I also let everyone die once just so I could see it. No ship upgrades, no loyalty, wrong squad choices. It was a slaughter, and Shepard died too.

Only jack wished miranda did though jack died because I thought she could handle the collectors and got shot in the stomach after the biotics part And I was so close too
will go back and correct this eventually

Zaeed died on me for the mission. He was the leader of the Fire Team. I thought that because he founded an ENTIRE MERCENARY GROUP that he might have some leadership skills.

No one else died. All my other choices were good. Garrus handled the Fire Team the other time, Tali went through the vents and Mordin took the team out. All of these decisions I thought were pretty logical.

I was incredibly angry when he died, on the principle of the matter.

Yeah, a shitload of collectors. My crew, however, suffered no losses. I was a good little player and had all loyalty, important ship upgrades, and I paid attention about who should be doing which jobs in the collector base.

First time around I lost Thane, Mordin and Miranda, granted I lost Miranda's loyalty and had no idea I get her loyalty back.

Thane seemed like a good choice for the ventilation bit and I sent Jacob back with the crew.

I lost two:
I sent Thane down the tubes, and I sent Zaeed as the second leader.

Yes, to be honest the reason he died would probably be Bioware forgetting to add him in, I mean he along with Garrus are pretty much the only two that are perfectly suited for the role!

On my first play-through, I actually tried to get everyone killed, including me. On the second run I managed to save everyone.

Finished the game twice, no one died. Although I kinda wanted them to, but hey, super paragon here, nicest guy on this side of the universe!

Zaeed died (don't care), as did Mordin (nooooooooooooooooooooooooo!) on my one and only playthrough.

Perfect saves are for pussies.

First playthrough? I had all upgrades and did all the loyalty missions, but Tali still managed to get herself shot in the head.

Second playthrough? Everyone. Everyone died. Except for Shepard. And Joker. But Joker doesn't count.
