Cut of a limb without passing out, wimping out, which is kind of difficult, going through bone and all. Perhaps the villian is a wizard and can magically remove it?
Medical assistance so that I do not die of blood loss

I would like to think that I would. In more detail, assuming I think the victim is a nice person, though I somehow doubt I would, in my cynicism of the self. I would definately do it for someone I knew and was close to. Also, for those who wish to know, I would take of my left leg. Prosthetic legs are much better than prosthetic arms. Unless we have the technology for perfectly functioning cool cyborg arms.

Here's the thing:if after you let the other guy die,the villain releases you,no one will blame you for not hacking a limb off.It's self-preservation and no one could predict the villain would actually keep his word,on account of being a lunatic and all.

If I can get a cybernetic one that is fully functional and connected directly to the nerve bundle of the lost limb - Yes.

No... I don't like people to begin with, why would I destroy my body to save something I don't like...
I don't think I'd even break a bone to save a stranger. Cutting off a limb is more likely to be fatal than not, so you might as well ask if we'd die to save a stranger. I suspect most people don't even have the willpower to seriously injure themselves on purpose, myself included. You should relaunch this one as a thread and ask for a few different degrees of injury one might accept in exchange for saving a stranger.
I could probably lose a foot to save someones life considering how much more awesome a pirate costume would be with a genuine pegleg.

The thing is if you cut off your own limb without almost immediate medical attention then the chances are quite high that you will bleed to death, let alone suffer immense cardiac trauma from the shock (not counting that guy who removed his own arm to escape a boulder, that was a fluke due to circumstances and conditions) So, this question is basically would you kill yourself to save another life.
well i dont really need a pinky toe and that counts as a limb so yeah if that counts sure, if were talking arms and legs and uhm ... other appendages then likely not
