Eternal Darkness is genuinely very creepy.
ETA - Haunting Ground scared me because I was always worried for the dog and Fiona.

Can't say there were many, but I can name a couple:

Zelda II: The final palace in parts where gigantic Bots would drop out of the ceiling when you passed under. I was 9 at the time though but I still go slowly through those areas.

Castlevania Legacy of Darkness: The Villa level had a jump scare that got me in one cutscene as your character explores a certain room (not talking about first entering the house though). That whole level was creepy (the music helped that).

Eternal Darkness. Just got playing through a few hours of the game without saving during a thunder storm. Then suddenly the sanity effect where it fakes a system crash went into effect....
Worst possible time.

I always find the inside of the moon on LoZ Majora's Mask very creepy. It's just so...unexpected...

I would say the Resident Evil Remake. Man that game is quality. Also, the sewer level of Shadows of the Empire. When I was a kid, I had to get my cousin to finish the level for me because I was too scared.

Certain moments in the Stalker games. Jesus, a bloodsucker jumping out of thin air in front of me always makes me scared.

The hotel level from Vampire the Masquerade: Bloodlines. I don't think I have ever been that much on edge. I even jumped at a toy horse.

Resident Evil 2. Combination of being 12 at the time, and the concept of a horrible, gory, post-apocalyptic cityscape still being relatively unexplored in popular media.

The suggested incestuous undertones with William Berkin's monster and Sherry was unsettling as well. And it was the last game in which I felt the conventional zombies were still somewhat frightening.

Definitely a good choice, scrapping the more modern police station ideas that formed the original RE2 build for the more classical, gothic ones that ended up in the game.

Almost nothing scares me anymore, but there is always one thing guaranteed to make me jump.

Sometimes the Necromorphs you kill in Dead Space aren't quite dead. You walk towards one you've just been firing at to scrounge for some loot, and BAM. Up it gets, screaming like crazy and nomming on your leg. Fuckers make me jump every time.

Yume Nikki is the most frightening game I've played in my life. It is the first game that literally left me sleepless for days. And I didn't even find Uboa. Yeah... I'm not one for jump-scare games.

The first time I ever had a sense of relentless terror while playing a game was when I was playing Mondo Agency. I found it very disturbing.

Silent Hill 2 was also pretty damn scary.

Devil May Cry.

What? Stop laughing. I'm serious! The first game had a terrifying atmosphere, and no amount of awesome-sauce sword-slinging killed that. Walking through a pitch-black castle, wondering which corner a marionette is going to strike from, is truly terrifying.

Oh, and of course, Silent Hill 2. Hearing that babies cry, seeing a torn-to-shreds body, then seeing Pyramid Head about 2 feet away from you, separated by nothing but bars was pretty damned scary.
