Most painful short term is probably when I dislocated my knee (apparently the most painful thing to dislocate), and putting it back in again myself - it's not right, being able to feel the two main leg bones join.
Long term is the time I ended up with around 20+ peptic ulcers and one 1.5cm duodenal - I lost three stone in two months and ended up needing three blood transfusions, completely missing xmas last year due to the week in hospital. I hadn't slept properly for the whole time, not helped by the fact that every single elderly person they put in the bed directly opposite me died.
Finally, the evil, evil nurses made me eat several bananas. Anyone who's had a mouth ulcer will tell you that that is not fun...
Oh, third place is when I tore my big toenail off - the blood and little straggly thready bits torn off were absolutely disgusting.
BTW - Anyone who has given birth absolutely wins this thread. I can't believe people actually want to do that on purpose. THINGS DISLOCATE AND TEAR. Wrong things.
