The final 6 minutes of the world is freaky as hell in Majora's Mask. Earthquakes, red sky, creepy music and that freaky moon. Probably wouldn't be as creepy if it didn't come out when I was 5.
Yeah, ALL of Majora's mask is pure unleaded nightmare fuel as a kid, even the music alone sends shivers down your spine.

Amnesia: The Dark Descent

The whole damn thing.

Another would be the underground sections of S.T.A.L.K.E.R, those places seriously shit me up the first few times I was down them.

The encounter with the Brood Mother in Dragon Age: Origins. Obviously they're incredibly grotesque, but it was just as freaky to find out just how the darkspawn are made.
Yeah, especially when right before you get there the voiceover plays out of nowhere. I was up at night and that part made me jump a bit.

Ravenholm wasn't scary for me, much more creepy and atomspheric.

On the other hand, the pitch black tunnel from Half Life 2: Episode 1 freaked me the hell out. I was low on ammo the entire time.

Any scene in Indigo Prophecy made me shudder, hide behind the couch, or make my Dad do it because I would be crying or hiding in the cabinet.

Anywho, I'm going to have the say the ending of Assassin's Creed Brotherhood was unnerving. I wasn't expecting it, and yes, I'm not going to spoil it.

Plus, after I replayed Bioshock, the part when Jack meets Andrew Ryan was unnerving as well...I especially the golf club scene.

For scariest? That's hard...I'm going to have to say when SHODAN reveals herself in System Shock 2. H o l y S h i t. After I got to that part, I couldn't sleep for days.

For me it would have to be the first time you run into a bloodsucker in stalker.
I was using headphones, so I wouldn't notice people behind me. My sister turned the light off in my room just when the bloodsucker appeared. I didn't shit bricks...I shat an entire house!

The second level of Vampire the Masquerade. Where you've got to go through that mansion. Holy fuckin shit. Here I was playing a nice fun vampire game and then you go and spoil it by making my bowels empty themselves all over the floor. On my second play through I had to turn the sound off and put my own music on.
Was right there with you Brother. I had a nasty case of the poopy pants all through that level. I think the thing that got me was the fact that you where a vampire! Overpowerd, imortal, top of the food chain. Your not afraid of anything apart from garlic, sunburn and religious nut-jobs right?..........Wrong!

A two headed skeleton seen at D-6 in Metro 2033. I don't know why, but It always seems to creep me out.

Another creepy mutant goes to Gil Alexander from Bioshock 2.

I've got a few, and for the most part, it's restating previously said ones.

Halo- The first encounter with the flood.

Condemned- The mannequins

Dead Space- That invulnerable thing that you have to freeze, can't remember what that was called, but that bit was intense

Quake IV- Being Stroggofied

Korsakovia- "The paramedics could not find his eyes. They believe he ingested them". DOWNLOAD IT NOW IF YOU OWN HL2!

Fear: Perseus Mandate- The bit after the nuke underground

Max Payne- The nightmares

Resident Evil 4- Beheading with chainsaws, Garrados, and the regenerators breathing.

Doom 3- Bloody corridor. Chainsaw Wielding demon. Apparitions. And then there's that bit with the mirror

Project Zero 2- When it gets in your face

Tomb Raider- Those fecking mummies, mainly because I was only 7

Fur Fighters- A mostly forgotten 3rdPS on the Dreamcast. The thing got weird when all the characters went to a hellish alternate reality. Was only 10 at the time, but can't find the game again.

I wouldn't say this was the scariest, but it definitely freaked me out. It's just so . . . unexpected. Very well done by Lionhead.
For some reason, I found the "Memory Lane" Demon Door from Fable 2 a bit creepy. Still sticking with the Fable games, although I didn't find them scary, there just something a little unsettling about the wide open desert areas in Aurora in Fable 3. I think it was just to do with the fact that compared to the busier areas in Albion, the desert just seemed so lonely by comparison.
