Oblivion was pretty scary for me in some parts.

For example,
In one of the Daedra quests, you have to go into the lair of an insane person and on one of the secret rooms you just find yourself surrounded by hanging corpses.

Scared the crap out of me.

Creepy: When a middleaged man askes your daughter to become his mistress in PM2.

Actually scary: When you turn around in minecraft and hear "sssh". Also the tentacles in HL, heart poundingly scary when you don't know what to do.

Fallout 1: The Glow
It's the... the radiation, the uncertainty of the whole thing, the background noises, the floor traps (made me shit myself multiple times) and the electrified door which I couldn't get past because I didn't think of using the BRIGHT YELLOW SECURITY KEY but instead I tried lockpicking, electronic-lockpicking, dynamite, a grenade, and a crowbar.

I would also like to add the Hospital level after the bombing in Brothers in Arms: Hell's Highway where

You suddenly find yourself alone in an abandoned hospital and you start seeing your friend's ghost, chairs moving suspiciously and one really odd bit where the world twists before you

You see now im very scared, im on the first few missions of HL2 and people are telling me the ravenholm part is scary. And i dont do scary. i mean fuck the first zombie scared me.

just how bad is it exactly?
It's a tricky question. I don't see Ravenholm as really being all THAT scary, so much as somewhat scary in an awesome way. It also doesn't leave so much to the imagination that average players will be horrified at length.

If you "don't do scary", though, do expect to jump a little when the undead break into the energy drinks xD

Actually, I think I might call it more "awesome" than "scary", though it has heaps of both. You'll understand why I mentioned the former attribute pretty early on in the mission!
Its mostly for those on the website which are 16-18 and played HL2 when it came out.. And even for them its just on the first Playthrough. And even then its only because of the buildup before. Just stick to the (Spoiler)Father and you will be alright.

I can think of several.

One would definitely be the Med Lab area in Dead Space. After seeing necromorphs bring dead bodies back as more necromorphs, I didn't go to the next area until I made sure I had smashed every single body in that room. The white sheets over their faces didn't help keep me calm, either. It was as if the game was telling me, "You don't need to know their faces; they'll change soon, anyway."

And then there's the Island in Resident Evil 4. At one point, you walk into a kind of hospital room and see what looks like a gray dead body on a hospital bed in another room. You go into the next room and find that the walls are completely covered with blood. You grab the card key and hear a loud bang from the previous room. The music changes, a labored breathing is heard from the other side of the door you just walked through, and then you see the body from before: the Regenerator. Nothing comes close to mowing down several dozen mooks and then finding out that this guy continues to walk towards you with a hole in his stomach from your shotgun, bearing razor sharp teeth and red eyes. Brrr...

Hearing this song when you start chapter 10 of dead space. You enter a room full of Unitology members committed suicide by shooting themselves in the head and you see bunch small candles that have burning for a while. You see writing on wall written in blood all the bodies are laying next to each other. And then you start hearing a woman's voice singing this song. Its so creepy
Just reading that paragraph made me shiver. I need to play Dead Space someday.
well I'm glad I was able to portray the atmosphere of dead space in my words. Its a great game you can find for 20 bucks new at your local gamestop or best buy.
Dammit, I should have picked that up yesterday. I got nearly every game I wanted but that.

Two bits in Silent Hill 3. The first bit was with that room in the hospital. "Oh, just an empty room with a mirror on the other side. Oh wait, what is that in the mirror. Oh crap. Dang it, the doors locked!". The other was in the church, when you have to get the tarot cards, and you go back to the original area. "Oh crap, what is that on the walls? Run away!!!"

Hmm, The jungle outside of where the Flood are in HALO: C.E. (That creeped me out - the faint movement and noise) and the ship in Black Ops. All of the dead Nazis, with grotesque disfigurements to the skin. Even though I knew what would happen I was scared a zombie would hurtle out of the darkness at me. (one Nazi even hung himself! It's like Der Riese radio messages again.)

The second level of Vampire the Masquerade. Where you've got to go through that mansion. Holy fuckin shit. Here I was playing a nice fun vampire game and then you go and spoil it by making my bowels empty themselves all over the floor. On my second play through I had to turn the sound off and put my own music on.
yes yes and yes,

also, the first time i played Bioshock 1 was at my freinds house at 11 pm untill 3 am, i stood up to go to the bathroom and i swear i was terrified a splicer would pop round the corner and attack me.

however, the most unerving thing goes not to a game but Dr Who, If they could figure out a way to put the Stone Angles in a game it would be impossible to stop playing and hold ur digestive system in tact

I remember playing Unreal I or II or whatever, I don't know in what game this was anymore but if anyone does, feel free to correct me.
The part at the beginning where you are chasing down the skaarj, but all you see is bodies and blood on the walls. At one part, a gate doesn't open completely, so you can't go through yet, and you hear the screams of the men being slaughtered inside. Doesn't look creepy now, but it sure had an effect on me back then (was about 8-9).

*Shudder*
I fucking hate the Skaarj for that.

In fact, all of Unreal was creepy. The Nali priests were the worst.

2 games caused me to scream my head off. the first was the original AvP when i got well acquaintened with a facehugger for the first time.

The other would be in F.E.A.R. 2, spoiler alert.

You're in the nuclear silo with the swing set, I walked up to it looked at the swing saw the teddy on the ground and heard alma's voice saying i love him. I then panicked looked over my right shoulder and SHE WAS THERE just 3 steps away as if she were looking over my shoulder that got a huge scream out of me.

More recently for just being slightly worried would be 1 of the demon doors in fable 3, spoiler.

A huge cavern with spider webs around, you get the treasure but hear a monsters cry ring out. I just had a reaction of yeah time to go very, very quickly before whatever the hell that is comes along.

Fallout 3's Dunwich building, especially the last part was more than a bit unsettling. At least for me.

And Majora's Mask had a fair bit of unnerving moments. I think in part because you just don't expect it from a Zelda game. Damn those alien bastards and their creepy theme song.
