Mine would be Silent Hill 3. That whole house museum freaked me out.

No game has ever really scared me, but I was definitely startled when a Truxican midget wrestler jumped out of a locker at me in Borderlands.

For me, it's fallout 3. How you can go ages wihtout meeting a single soul and then when you finally interact with someone its a creepy centaur in a dark deserted hospital *shudder*. STALKER games are pretty scary too, for the same reason.

Thief. Especially the mine level and the level after you lose your eye. Both scared the hell out of me. Granted, I was 12 at the time, but I've never encountered another game I found quite so scary.

I have the same fear with Fallout 3 and Oblivion too! When in the wilderness, sometimes I just spin around and make sure there isn't something there, there is that constant fear that something might always be RIGHT BEHIND ME!!!
